This background check may include, but is not limited to:

• Criminal history records from federal, state, and local agencies


• Sex offender registry records


• Identity verification


• Other public record information relevant to volunteer service

I understand that the information obtained will be used solely for volunteer screening purposes and will remain confidential within the leadership of the church.

I release FAC of Tyler, its officers, employees, and agents from any liability arising from the use of such information for legitimate screening purposes.

This authorization shall remain valid for the duration of my service unless revoked in writing.
